OHIP Card

It is important to have a valid OHIP card with you, when you come to the doctor's office.
In the clinic your OHIP card is used only for:

  • Patient identification
  • Confirmation of the correct spelling of your legal name
  • Verification of your OHIP details on the health care card with the details in our system

Prescription Refills

  • Typically your prescription should last until your next visit.
  • All prescriptions should be done in person.
  • We suggest you make an appointment about two weeks prior to your next refill.
  • Prescription refill without a visit to the doctor is not covered by OHIP. Additional fees may apply.

Make an Appointment

Registered Patients may book or review an appointment on-line at our medical portal or call us at 905.771.7755.

New (non-registered) Patients are encouraged to call us at 905.771.7755 and book an appointment during regular hours of operation. You are advised to fill out a Patient Questionnaire and take it to the first appointment with you.

For your first visit to the doctor please bring:

  • Medical summary of your records from your previous physician
  • List of your current medications
  • List of known allergies for medications and food
  • List of surgical procedures and hospital stays
  • Laboratory results for the past six months
  • Immunization records for the kids under six years old

Walk-in Patients may be seen by doctor or a physician assistant on a first-come first-served basis. The exam results are always discussed with the doctor.

For your convenience, we designed a special schedule for walk-in patients: Mon - Fri: 11.00 - 18.00. You are encouraged to call us at 905.771.7755 prior to your visit to check the doctor's availability. All emergencies will be given a priority.

Please note that walk-in patients are treated only for immediate health problems. Due to a limited time frame, regular health issues (prescription renewal, general exams, medical documents, mental health consultations, third-party physicals, routine care, etc.) cannot be addressed on a walk-in basis. During your visit, the doctor will assess your health concern and provide the appropriate treatment, which could include medications, further testing, a referral to a specialist, or a follow-up for a routine visit. Please book an appointment for all other medical concerns.

Lab appointments may be scheduled during the laboratory hours of operation over the phone (905.771.7755) or in person.

Scheduling

General checkup visit - up to 30 min. Regular visit - 15 min. Walk-in visit - 10 min.

During peak hours you may reach the answering machine. In this case please leave us your name, phone number and a brief message. We will return your call within 2 business days.

According to our clinic policy, we can address no more than two concerns during a regular appointment. If you have more than two concerns, please book a follow-up appointment or ask the secretary for additional time.

We understand that your time is important and our goal is to stay on schedule, however, an emergency situation may arise; as a result, your waiting time may exceed 45 minutes. We apologize for the inconvenience and appreciate your understanding. There is a zero-tolerance policy for any harassment, intimidation or abuse of any member of our staff or physicians.

Cancellation Policy

If you are unable to attend an appointment please be courteous and call us at least 48 hours before the appointment. Appointments are in high demand, and your early cancellation will give another person the possibility to have access to timely medical care. Missed appointments without cancellation notice will be recorded in the patients' chart as "No show". Patients with more than one such occurrence will be charged as follows: the first time - the patient will be reminded about this clinic policy. The second time and third time - the patient will be charged $35 for a regular appointment and $90 for a physical exam, small procedure or counseling appointment. Any patient who has more than 3 missed appointments and/or cancellations with less than 48 hours' notice will be seen on a walk-in basis only.
